Direct Services and Case Management Support
Tailored Day Services (TDS) is essentially an individualized Day Program designed for adults to receive instructional and functional skills training in vocational, educational, and community-based environments to commence or maintain a sustainable and independent role in their community
Clients will have a team of direct service professionals/caseworkers including a representative of the San Diego Regional Center to create an Individual Support Plan (ISP) to state/outline their individual goals and maintain record of individual progress. A direct service professional/caseworker is assigned to the client to meet on a weekly basis 1:1 to work on the life goals outlined for the client and record the progress consistently in order to present to the team on a semi-annual basis for review and updating of the client’s individual support plan.
The curriculum of TDSO services includes, but is not limited to:
Instructional/Functional Skills Training:
1:1 Support @ 32-37 hours per month (personalized to meet client’s needs)
Vocational Development
Competitive Integrated Employment
Paid Internship
Educational Development, supports & resources
Shopping in natural environments
Money management/budgeting
Public Transportation/Mobility Training/Safety Skills
Self-care/personal hygiene
Self-Advocacy/Communication
Social Recreation/Community integration
Other Services and resources:
Social Impact Services partners with community organizations, subsidy programs, and licensed professionals that may provide additional support to a client given the nature of support required.
Case Management:
Planning team meetings on a quarterly basis with the client, support systems and SDRC
ISP Reports detailing initial, daily, semi-annual, and annual progress/notes
Medical Forms detailing medical history and medications
Process to become a client:
Clients of interest must be receiving services/funding from San Diego Regional Center (SDRC). If the client of interest is not receiving services and would like to begin the intake process with SDRC, they may visit https://www.sdrc.org/apply for additional information. Once the client of interest is approved with the San Diego Regional Center for services and/or has a planning team meeting with SDRC to discuss his/her interest in this program with their assigned San Diego Regional Center Service Coordinator and/or the Program Manager of their assigned unit should their case be temporarily unassigned, the Service Coordinator/Program Manager may contact Social Impact Services directly for a referral.
