Adult Day Program

Direct Services and Case Management Support

Day Program is a community-based and group-setting program at a 1:3 ratio.

Day Program is designed for transitioning aged youth (18-22) and adults to receive instructional and functional skills training in vocational, educational, public transportation, and recreational environments to commence or maintain a sustainable and independent role in their community.  This program is a Monday through Friday schedule with a list of activities and roles to fulfill that may prepare them for community independence involving paid employment and/or a smooth transition from a familiar San Diego Unified School District program, TRACE (intended for reference not formality). Clients will have a team of direct service professionals/caseworkers including a representative of the San Diego Regional Center to create an Individual Support  Plan (ISP) to state/outline their individual goals and maintain a record of individual progress. A  direct service professional/caseworker is assigned to the client to meet on a daily, Monday through Friday basis to work on the life goals outlined for the client in a group setting and record the individual progress consistently in order to present to the team on a semi-annual basis for review and updating of the client’s individual support plan.
